What is Group Care?

Group care brings a small group of patients together with a doctor to address a shared condition. The extended time and laid-back setting of the group make for an innovative experience to complement your normal primary care. Regular meetings facilitate deep exploration of a topic, and participants benefit from the shared experience and support of their peers.

We are planning groups on a variety of medical topics starting this Summer/Fall with Hypertension and Insomnia. Our Group Care doctors bring their medical and behavioral expertise to the patient group setting, providing individual and up-to-date recommendations on important medical topics.

  • The goal of our 4-session group intensive is to empower women to move through menopause and beyond with more ease, health and understanding. This group is open to any woman - whether you're many years away from menopause, are currently in the thick of it, or have already moved through it.

  • We believe that living well includes thoughtful planning for the future. The Living Well, Dying Well Group is designed to help you navigate important topics including advanced care planning, living wills, palliative vs hospice care, how to start difficult conversations about end of life.

Group medical visits are billed to your insurance, just like any other visit to your doctor, and will be based on the length of each Group Visits. There is no extra fee for participation, though you may be charged an additional payment based on your co-payment, co-insurance or plan deductible similar to any other visit to our office.

Past Groups

  • Our insomnia group (sessions completed) offered the gold-standard treatment for insomnia, Cognitive Behavioral Therapy for Insomnia, and more! Dr. Michelle Jonelis, an experienced behavioral sleep medicine clinician, brought together cognitive behavioral therapy for insomnia, acceptance and commitment therapy for insomnia, and lifestyle medicine to teach the skills needed to fix sleep problems, permanently.

  • Had you ever wished for more time to discuss with your medical provider? Last year, our hypertension group provided a small community setting for participants to dive deep into hypertension alongside others who shared similar experiences. Led by our new Panoramic physician, Dr. Angela Mendoza, a skilled group leader and dedicated teacher of healthy living, the group presented the case for medication and lifestyle improvements, offering individual treatment recommendations for each participant.
